How Can I Schedule An Appointment?
Patients that want to visit a physician at Park Avenue Endocrinology & Nutrition must schedule an appointment. This can be done online or by calling our office at 212-772-7628. Before your call please be familiar your insurance information such as policy, copays, and deductible fees in advance and if your plan requires an insurance referral from a primary care provider.
We ask patients that cannot make their appointment provide us with 24 hours notice of cancellation. We provide ample time for each patient, however, we do not refuse emergencies. Therefore, our office schedule can be disrupted at times. Please be patient with these changes as a priority for emergency care ensures the highest quality care for all of our patients.
Park Avenue Endocrinology & Nutrition is open Monday – Thursday from 9 AM to 5 PM and on Fridays from 9 AM to 1 PM. For more information about hours and to schedule a consultation please call our office!
In most cases, we accept Medicare assignment and private insurance will be accepted. A member of our team can provide patients with more information and verify if your plan is accepted. Please be aware that if your insurance requires a referral from a primary care physician to be seen by our endocrinology specialists, you must arrange that before the date or your appointment.
In addition, please be aware of your copay and in-network deductible prior to your appointment. We are happy to accept flex or HAS cards, etc. At the time of your appointment, we will need pertinent information including the policy holder’s name, contact number and, if applicable, group number.
Prescriptions, Refills & Lab Results
For prescription refills, please call the office during office hours. Our staff checks messages periodically throughout the day and every effort will be made to refill prescriptions by day’s end. We ask that you please allow 1-2 business days depending on the time of your call and the nature of your prescription. For insurance appeals, prior authorization, or medical necessity letter please allow 1-2 weeks.
Lab results with changes to regimens will be mailed or e-mailed to patients and you will be called if your lab results require immediate attention. Patients concerned or have questions about labs can call our front desk during office hours.
To maintain quality care at a reasonable cost and to minimize the cost of billing and collection, we ask that you pay for services as they are rendered.
